RELIGIOUS EDUCATION REGISTRATION INFORMATION

FAMILIES PLEASE COMPLETE THE FOLLOWING FORM

PLEASE FILL OUT
ONE FORM FOR EACH CHILD

Any students who are already confirmed,
but have not yet graduated from high school
AND are under 19-years-of-age,
are invited to participate in the Upper Room Youth Group,
which seeks to continue a religious education experience
within a fraternal setting until your child finishes high school.

Participation is at the discretion of each household. 

Parent volunteers are welcomed to assist at any level upon completion

of the required Safe Environment qualifications
and in coordination with the Religious Education Team.
